WHEN YOU CALL IN PERSON VISITS REMEMBER TO TELL US ...

 

Service Tips is information that may help you access your information faster. It tells you what to have ready when you call us or visit

us either in person or on line. It reminds you of the things that you need to tell us. Just click on the pictures above. It will give you

information about keeping your records up to date so that we can serve you better.

 

WHEN YOU CALL

Information We Need When You Call:

  • Is your question about medical, dental, vision, prescription or life?
  • Member's social security number or alternate ID Number
  • (found on a member's Explanation of Benefits)
  • Member's present/past Employer
  • Member's group number? (Found on your ID number
  • Patient and what is their relationship to the member
  • Type of service are you asking about.
  • Be specific! For instance, if you ask what your Plan pays for a doctor's office visit when you're going to the Chiropractor, you may not
  • find out that your Plan has a list of Chiropractors that you must use in order to have coverage.

 

IN PERSON VISITS
You Want to Visit Us in Person:

Our Benefit Information Center in Alhambra, San Diego and Las Vegas are open between 8:30 a.m. and 4:00 p.m. Monday through

Friday. The Benefit Adviser will be more than happy to assist you with your Health & Welfare inquiries.

Walk-ins are welcomed. You can avoid any wait by making an appointment ahead of your visit. We can have your information ready

so that we can serve you more efficiently. The number to call for an appointment is (626) 299-5897 or toll free at (877) 350-4792,

extension 897 for Alhambra, (619) 287-1043 for San Diego, or (702) 252-7001 for Las Vegas.

 

ON LINE

You Want to Visit Us On Line:

Accessing your benefit information on line just got easier. Register as a member or a dependent and we will be able to give your

current eligibility, claims that have been paid under your medical or dental plan. We even give you a quick summary of your benefits

in an easy to print format that you can keep for your reference or take with you to your doctor.

 

REMEMBER TO TELL US ...
Things You Need to Remember to Tell Us About:

  • Address Changes
  • Marriage or Divorce
  • Dependent children or stepchildren that need to be added or deleted
  • Death in the Family
  • Refer to the Life Events page or click on the appropriate underlined word above.
